Who is eligible for county care in Illinois?

CountyCare eligibility
Be 19-64 years old. Have yearly income at or below 133% of the Federal Poverty Level. Not be eligible for Medicaid. Not be eligible for Medicare.

How do I choose a Medicaid plan in Illinois?

An individual must call the Client Enrollment Broker Call Center at 1-877-912-8880 (TTY: 1-866-565-8576) or go online to the Enrollment Portal at www.enrollhfs.illinois.gov to get more information about their HealthChoice Illinois plan choices and to make a plan switch.

Is County care only Cook County?

No. At this time, CountyCare coverage is only available to Cook County residents. Network and Services Can I keep my current doctor if I enroll into CountyCare? You may be able to keep your current doctor if they are part of the CountyCare provider network.

How do I cancel my county care in Illinois?

You can complete your REDE one of three ways:
