How do I find MyCHOP access code?

Please call your CHOP healthcare provider's office or request MyCHOP help. You will be given a new access code to sign in to MyCHOP. Once you have entered your new access code along with your date of birth, your original MyCHOP username will appear. You will then be asked to select a new password.
